Records fall into three groups:
\\n
\\n
Peer-reviewed studies, including cell, animal and human research and systematic reviews. We note the model, such as \"in rats\", because early findings often do not carry over to people.
\\n
Trial registrations and results, including trials that did not meet their primary endpoints.
\\n
Regulator records from the US Food and Drug Administration (FDA) and other bodies, plus sport listings such as the World Anti-Doping Agency Prohibited List.
\\n
\\n
Compounds covered include BPC-157, TB-500, MOTS-c, SS-31 (elamipretide), Semax and DSIP (emideltide). For several of these, including BPC-157, TB-500 and MOTS-c, the published evidence is mainly preclinical. SS-31 is an exception: it has been studied in clinical trials and, as elamipretide, received an FDA accelerated approval for one rare disease.
\\n
How items are sourced and dated
\\n
Every item links to a primary record: a journal abstract, a registry entry or a regulator\\'s own page. Updates that cannot be traced to such a record are left out. Each item shows the date of the record itself and the date its status was last checked, because regulatory status can change.
\\n
Latest signals
\\n
\\n
CagriSema phase 3: the REDEFINE trials. Novo Nordisk\\'s REDEFINE programme has published three phase 3 results for CagriSema, the cagrilintide–semaglutide combination: REDEFINE 1 and 2 in the New England Journal of Medicine (PubMed) and REDEFINE 5 in Japan and Taiwan, while the October 2026 design paper for the REDEFINE 3 cardiovascular outcomes trial describes 7,076 randomised adults. CagriSema remains investigational. Record date: 6 October 2026. Status last checked: 25 October 2026.
\\n
Thymosin alpha-1 in sepsis: the TESTS trial. The phase 3 TESTS trial of thymosin alpha-1 in 1,106 adults with sepsis, published in The BMJ (PubMed), found 28-day mortality of 23.4% versus 24.1% with placebo (hazard ratio 0.99), a neutral result after the smaller ETASS trial had pointed to a lower rate. Thymosin alpha-1 is not an FDA-approved drug. Record date: 15 January 2025. Status last checked: 24 October 2026.
\\n
WADA Prohibited List and research peptides. The World Anti-Doping Agency Prohibited List prohibits BPC-157 explicitly under Category S0 (Non-Approved Substances) and thymosin beta-4 derivatives such as TB-500 under Category S2.3 at all times, while recent UHPLC-HRMS studies map their cleavage metabolites in urine and dried blood spots (2026 Analyst workflow, PubMed). Record checked: 23 October 2026.
\\n
Retatrutide phase 3 papers. On 29 September 2026, the TRIUMPH-1 trial of retatrutide in adults with obesity was published in the New England Journal of Medicine (PubMed) and TRIUMPH-2, in adults with obesity and type 2 diabetes, in The Lancet (PubMed). Both 80-week, placebo-controlled phase 3 trials were funded by Eli Lilly. Retatrutide remains investigational. Record date: 29 September 2026. Separately, FDA\\'s page of bulk drug substances that may present significant safety risks lists BPC-157 and \"thymosin beta-4, fragment (LKKTETQ), also known as TB-500\" among substances whose category 2 nominations were withdrawn. Emideltide (DSIP) is also listed on that FDA page. Record dates: 23–24 July 2026 (meeting). Status last checked: 19 October 2026.
\\n
Elamipretide accelerated approval. On 19 September 2025, FDA granted accelerated approval to Forzinity (elamipretide) for Barth syndrome. According to FDA, the approval was based on an intermediate endpoint, and a confirmatory randomised, placebo-controlled trial is required. Research-grade SS-31 is not the approved product and is not covered by the approval. Record date: 19 September 2025. Sources are linked for verification.
\\n
What KlowSignal is
\\n
KlowSignal tracks new studies and regulatory updates on research peptides. An independent editorial Research Desk runs the site. Each page reports what a primary record says and links to it. Some linked records describe animal studies, human trials or regulatory approvals. Nothing here is medical advice. Nothing here recommends, promotes or gives instructions for the use of any compound in people or animals.
\\n
What counts as a signal
\\n
A signal is a new or changed entry in a primary record:
\\n
\\n
PubMed records, such as the 2026 systematic review by Poolton et al..
\\n
ClinicalTrials.gov registrations and results.
\\n
FDA documents, such as advisory committee meeting pages.
\\n
Company filings, reported as the company\\'s own statements.
\\n
\\n
News coverage, forums and marketing copy are not signals. Claims that cannot be traced to a primary record are left out. The guide How to read a research signal explains the approach in more detail. Examples of signal reports include the July 2026 FDA compounding committee and the elamipretide accelerated approval.
\\n
Dating and updates
\\n
Each article gives the date of the record it reports and the date its sources were last checked. When a source changes, a dated update note is added to the article. Statements that something has not been found, such as the absence of a marketing authorisation, always carry an \"as of\" date.
\\n
Where wording on a page differs from a linked primary source, the primary source takes precedence. This also applies to any translated version of a page: the original English text and the linked primary records are the reference versions.
\\n
Corrections
\\n
Errors can be reported through the contact address below. Please include the page address and the primary source that shows the problem. Confirmed errors are corrected, and the change is dated on the page.
\\n
Paid links
\\n
Some pages carry paid links to Purita, a research-use-only supplier. KlowSignal is paid when readers follow these links. They are not an endorsement and do not influence which records are reported or how they are described. Research-use-only material is not an approved medicine and has not been evaluated by regulators for use in people. Full details are in the affiliate disclosure.
\\n
Contact
\\n
Questions, corrections and source suggestions can be sent to the Research Desk at info@zvezdaff.com. Sources are linked for verification.
\\n
Why this guide exists
\\n
KlowSignal reports new studies and regulatory updates on research peptides. A mouse study, a company announcement and an FDA meeting notice can all mention the same compound and still say very different things. This guide explains how we read each type of record. More about the site is on the about page.
\\n
Study types and levels of evidence
\\n
\\n
Cell studies (in vitro). These show whether a compound can affect a pathway under controlled conditions. Birk et al. (2013) reported that SS-31 binds the mitochondrial lipid cardiolipin with high affinity. This describes a mechanism, not what happens in a whole organism.
\\n
Animal studies (in vivo). Lee et al. (2015) reported effects of MOTS-c on insulin sensitivity in mice. Results in rodents often do not carry over to people, and work from a single laboratory needs independent replication.
\\n
Observational human studies. These compare existing measures, such as natural blood levels of a peptide, across groups. They show associations, not cause and effect.
\\n
Phase 1 trials. Usually small studies of safety, tolerability and how the body handles a compound.
\\n
Phase 2 trials. Larger studies looking for an early sign of effect and gathering more safety data.
\\n
Phase 3 trials. Large, usually randomised, placebo-controlled studies designed to confirm an effect. They can fail: MMPOWER-3, a phase 3 trial of elamipretide with 218 participants reported by Karaa et al. (2023), did not meet its primary endpoints.
\\n
Systematic reviews. These gather and grade all studies on a question. Poolton et al. (2026) found three small human studies of BPC-157 with 30 participants in total and rated the certainty of evidence as very low.
\\n
\\n
A primary endpoint is the main outcome a trial is designed to test. Missing it means the trial did not show what it set out to measure.
\\n
Press release, peer-reviewed paper or registry entry
\\n
\\n
Press release. Written by a company or institution, often the first word on a result such as \"positive topline results\". It is selective, usually lacks full methods and has not been independently checked.
\\n
Peer-reviewed paper. Assessed by other researchers before publication and expected to report methods, numbers and limitations. Peer review is a filter, not a guarantee.
\\n
Trial registry entry. A public record of a trial\\'s design, planned outcomes and status. Comparing it with the final paper can reveal changed or unreported outcomes. It does not show that any result exists.
\\n
\\n
FDA vocabulary
\\n
\\n
Approval. FDA has authorised one product, from one sponsor, for the use described in its label. It does not extend to other material containing a similar molecule.
\\n
Accelerated approval. Approval based on an intermediate or surrogate endpoint, with a required confirmatory trial. As covered in our elamipretide article, FDA granted accelerated approval to Forzinity (elamipretide) for Barth syndrome on 19 September 2025, based on knee extensor muscle strength. Research-grade SS-31 is not that product.
\\n
503A bulks list. Bulk drug substances that pharmacies compounding under section 503A may use. Being considered for the list is not being on it.
\\n
Category 2. Under FDA\\'s interim policies, nominated substances that may present significant safety risks are placed in category 2, with the section concerned, the date added and the risks identified.
\\n
Nominated but withdrawn. The same page lists substances previously in category 2 whose nominations were withdrawn, including AOD-9604, BPC-157 and TB-500. For BPC-157, FDA noted possible immunogenicity, peptide-related impurities and no, or only limited, safety-related information. Withdrawal is not a finding of safety.
\\n
Pharmacy Compounding Advisory Committee (PCAC). An expert committee that advises FDA; FDA decides. The July 23–24, 2026 meeting page lists BPC-157, KPV, TB-500, MOTs-C, emideltide (DSIP), Semax and Epitalon as being considered for the 503A bulks list, with the uses FDA reviewed, such as ulcerative colitis for BPC-157. Uses evaluated are not established effects. See our meeting coverage.
\\n
\\n
What research-use-only labelling means
\\n
A research-use-only (RUO) label means a material is sold as a laboratory reagent for non-clinical research. It is not a medicine, has not been evaluated or approved by FDA, and is not intended for human or animal use. The label says nothing about whether a compound works. For laboratories the key questions are identity and purity, and batch-level certificates of analysis are the minimum documentation to expect.
\\n
Checklist for each signal
\\n
\\n
\\n
\\n
Signal
\\n
Can show
\\n
Cannot show
\\n
Questions we ask
\\n
\\n
\\n
\\n
\\n
Cell study
\\n
A possible mechanism
\\n
Effects in organisms or people
\\n
Which cells? If that is not possible, the material will be made available at the meeting location and on FDA\\'s website at the time of the meeting. The background documents, once posted, are where FDA\\'s own assessment of each substance will appear. This article does not summarise them.
\\n
Public docket
\\n
According to the meeting page, FDA has established a docket for public comment:
\\n
\\n
Docket number: FDA-2025-N-6895
\\n
Closing date: 22 July 2026
\\n
\\n
The page states that interested persons may present data, information or views, orally or in writing, on issues pending before the committee. It also notes that late or untimely comments are handled differently; the exact terms are on the FDA page.
\\n
Context: the category 2 list
\\n
Separately, FDA maintains a page on bulk drug substances used in compounding that may present significant safety risks. FDA explains there that it identified potential significant safety risks when reviewing nominations for the 503A or 503B bulks lists, and that such substances \"have been placed in category 2 under the interim policies\".
\\n
That page also lists substances previously in category 2 whose nominations were withdrawn by the nominators. BPC-157 appears in that section. FDA\\'s stated concerns were that compounded drugs containing it \"may pose risk for immunogenicity for certain routes of administration and may have complexities with regard to peptide-related impurities and active pharmaceutical ingredient (API) characterization\", and that the agency \"lacks sufficient information to know whether the drug would cause harm when administered to humans\". The same withdrawn section lists thymosin beta-4 fragment (LKKTETQ), also known as TB-500.
\\n
FDA\\'s category 2 page also lists emideltide (DSIP), which is on the 24 July agenda. This article does not state which part of that page the entry sits in; the current wording and status should be checked on the FDA page itself.
\\n
The meeting page does not describe how the July 2026 review relates to these earlier category 2 entries. Readers interested in that connection should consult FDA\\'s records directly rather than infer an outcome.
\\n
What the record does and does not show
\\n
The meeting page establishes the dates, the seven substance groups, the salt forms named, the uses FDA reviewed, the invitation to nominators and the docket details. It does not indicate how the committee will advise FDA, what FDA will decide, or how strong the scientific evidence for any listed use is.
\\n
For several of these substances the published research base is limited. Much of the BPC-157 literature comes from rat studies by a single research group, published work on TB-500 as a distinct compound is largely analytical chemistry for doping control, and human studies of MOTs-C are mostly observational. A listed \"use evaluated\" reflects the scope of a regulatory review, not an established finding.
\\n
For a general approach to regulatory items like this one, see our guide on how to read a research signal. For a contrasting example, see our report on elamipretide\\'s accelerated approval: a peptide that received FDA accelerated approval for one rare disease, based on an intermediate endpoint and conditional on a confirmatory trial.
\\n
Where to check for updates
\\n
The authoritative source for the agenda, materials, any change of schedule and the meeting outcome is the FDA meeting page. It belongs to the Szeto-Schiller (SS) peptides, named after Hazel Szeto and Peter Schiller, who developed the series.
\\n
The structure alternates aromatic and positively charged residues. According to the research literature, this pattern lets the peptide cross cell membranes and concentrate in the inner mitochondrial membrane.
\\n
The SS peptides were first developed in opioid receptor research. Researchers later noticed that some members of the series accumulated in mitochondria, and SS-31 became the lead compound for that work. A 2014 review by Szeto in the British Journal of Pharmacology describes the discovery and development of SS-31 as the first compound designed to interact with cardiolipin.
\\n
Cardiolipin: the laboratory findings
\\n
Cardiolipin is a phospholipid with four fatty-acid chains. It is found almost only in the inner mitochondrial membrane, where it shapes the folds known as cristae and helps organise the respiratory chain. The preclinical record on SS-31 centres on this lipid:
\\n
\\n
Binding. Using a fluorescent analogue, Birk et al. (2013) reported that SS-31 binds cardiolipin with high affinity.
\\n
Cytochrome c. The same work reported that the SS-31-cardiolipin complex inhibited the peroxidase activity of cytochrome c, a reaction that damages cardiolipin under stress.
\\n
\\n
These are laboratory findings about mitochondrial structure and chemistry. Results in cell-free systems, cultured cells or animal models do not predict outcomes in other conditions or in people.
\\n
The clinical record before approval
\\n
SS-31 is unusual among the compounds covered on this site because it has been through formal clinical development, and that record includes a negative result.
\\n
\\n
Primary mitochondrial myopathy. A randomised crossover trial in adults by Karaa et al. (2020) was followed by MMPOWER-3, a phase 3 randomised placebo-controlled trial reported by Karaa et al. (2023) with 218 participants. MMPOWER-3 did not meet its primary endpoints, the six-minute walk test and a fatigue score.
\\n
Barth syndrome. Barth syndrome is a rare genetic disease of cardiolipin metabolism. Smaller trials in this condition formed the basis of the 2025 FDA decision.
\\n
\\n
A trial that misses its primary endpoint has not shown the effect it set out to measure. That result sits alongside the later approval in a different, much narrower setting.
\\n
The 19 September 2025 decision
\\n
On 19 September 2025, FDA granted accelerated approval to Forzinity (elamipretide) for Barth syndrome, limited to people weighing at least 30 kg.
\\n
Two conditions shape how the decision should be read:
\\n
\\n
An intermediate endpoint. The approval was based on knee extensor muscle strength, an intermediate measure rather than a direct measure of long-term clinical outcome.
\\n
A confirmatory trial. FDA required a confirmatory randomised, placebo-controlled trial.
\\n
\\n
The primary records are FDA\\'s approval document, the Drug Trials Snapshot for Forzinity and the prescribing information. Readers who want the regulator\\'s own wording should go to these documents rather than to secondary summaries.
\\n
What accelerated approval means here
\\n
Accelerated approval is an FDA pathway that allows approval on the basis of an intermediate or surrogate endpoint, on the condition that a confirmatory trial follows. Three points follow from the record:
\\n
\\n
The approval is conditional on the confirmatory trial FDA required.
\\n
The approval is narrow: one disease and a body-weight limit.
\\n
The approval does not settle questions in other conditions. The MMPOWER-3 result remains part of the record.
\\n
\\n
Our guide on how to read a research signal explains why intermediate endpoints and conditional approvals need careful reading.
\\n
Why the approval does not extend to research-grade material
\\n
A prescription approval applies to a specific product: one manufacturer\\'s material, made to a defined specification, with a reviewed label, for a stated indication. It does not attach to the molecule in general.
\\n
SS-31 sold as a laboratory reagent is not Forzinity. It has not been evaluated by FDA, it is not covered by the approval, and the approval says nothing about its identity or purity. For laboratory work, batch-level certificates of analysis are the minimum documentation a laboratory should expect, because product identity and purity affect whether results can be reproduced.
\\n
Elamipretide does not appear on the agenda of FDA\\'s July 2026 Pharmacy Compounding Advisory Committee meeting, which covers other peptides nominated for the 503A bulks list. We found no EU marketing authorisation for elamipretide as of 19 October 2026.
\\n
Open research questions
\\n
\\n
How cardiolipin binding translates into measurable changes in tissue function across conditions.
\\n
Whether findings in Barth syndrome, a disease of cardiolipin itself, apply to other mitochondrial disorders.
\\n
How long-term exposure affects mitochondrial membranes and cardiolipin content in animal models.
\\n
What the required confirmatory trial will report.
\\n
\\n
Limitations of the evidence
\\n
\\n
The phase 3 trial in primary mitochondrial myopathy did not meet its primary endpoints.
\\n
The Barth syndrome approval rests on an intermediate endpoint and is conditional on a confirmatory trial.
\\n
Preclinical findings in laboratory and animal models do not predict outcomes in other conditions.
\\n
\\n
Key terms
\\n
\\n
Cardiolipin. A phospholipid with four fatty-acid chains, found almost only in the inner mitochondrial membrane.
\\n
Cristae. The folds of the inner mitochondrial membrane, where much of the respiratory chain sits.
\\n
Intermediate endpoint. A measure taken as an early indication of an outcome, rather than the outcome itself.
\\n
Primary endpoint. The main outcome a trial is designed to test.
\\n
\\n
Where researchers source it
\\n
Purita is a store that lists SS-31 for laboratory research use only. A search of FDA\\'s drug approvals data (Drugs at FDA) through the openFDA interface on 22 October 2026 returned no record for retatrutide.
\\n
\\n
What was published
\\n
Retatrutide is described in both papers as an agonist of the GIP, GLP-1 and glucagon receptors, which is why it is often called a \"triple agonist\". Its earlier phase 2 obesity trial, a 48-week study of 338 adults, was published in 2023 (Jastreboff et al., 2023, PubMed).
\\n
The two new papers are the first full peer-reviewed reports from the TRIUMPH phase 3 programme in obesity:
\\n
\\n
TRIUMPH-1 (Jastreboff et al., 2026, PubMed; registry entry NCT05929066) enrolled adults with obesity who did not have type 2 diabetes.
\\n
TRIUMPH-2 (Bellido et al., 2026, PubMed; registry entry NCT05929079) enrolled adults with obesity or overweight and type 2 diabetes.
\\n
\\n
Both were randomised, double-blind and placebo-controlled, ran for 80 weeks and compared three retatrutide groups with placebo. ClinicalTrials.gov lists both trials as completed.
\\n
TRIUMPH-1: obesity without diabetes
\\n
TRIUMPH-1 randomised 2,339 participants. It had three primary outcomes, each tested in the two upper retatrutide groups against placebo:
\\n
\\n
percent change in body weight in the whole trial population;
\\n
change in the WOMAC knee pain score in a subgroup of 574 participants with knee osteoarthritis;
\\n
change in the apnoea-hypopnoea index, a count of breathing interruptions per hour of sleep, in a subgroup of 243 participants with obstructive sleep apnoea.
\\n
\\n
According to the abstract, mean body-weight change at 80 weeks was −17.6%, −23.7% and −25.0% across the three retatrutide groups, compared with −3.9% with placebo. The authors report that both primary comparisons were statistically significant (P<0.001). The knee pain and sleep apnoea comparisons were also reported as significant in both of the analyses the paper presents. The most common adverse events were gastrointestinal.
\\n
TRIUMPH-2: obesity with type 2 diabetes
\\n
TRIUMPH-2 randomised 1,152 participants at 92 centres in eight countries, and 965 (84%) completed the study drug. The table summarises figures stated in the abstract.
\\n
\\n
\\n
\\n
Measure (week 80)
\\n
Retatrutide groups
\\n
Placebo
\\n
\\n
\\n
\\n
\\n
Mean body-weight change, treatment-regimen estimand
\\n
−11.9%, −16.8%, −18.8%
\\n
−5.1%
\\n
\\n
\\n
Diarrhoea
\\n
27%, 34%, 34%
\\n
13%
\\n
\\n
\\n
Nausea
\\n
14%, 21%, 28%
\\n
8%
\\n
\\n
\\n
Hypotension
\\n
1%, 5%, 6%
\\n
under 1%
\\n
\\n
\\n
Dysaesthesia (abnormal skin sensation)
\\n
4%, 6%, 7%
\\n
1%
\\n
\\n
\\n
\\n
The authors also report improvements in glycaemic control. Permanent discontinuation of the study drug because of adverse events or death was more frequent in the two upper retatrutide groups (12% and 8%) than in the lowest group (4%) or with placebo (5%). Seven participants died during the trial, six in the retatrutide groups and one in the placebo group, and the investigators judged all deaths unrelated to the study intervention.
\\n
Why the two papers report different kinds of estimate
\\n
Large trials have to decide how to count participants who stop the study drug or start other measures. The rule chosen is called an estimand.
\\n
\\n
The treatment-regimen estimand, used for the weight outcomes in both papers, counts every randomised participant whether or not they kept taking the study drug. It is close to what trial reports call an ITT analysis, in which participants are analysed in the group they were randomised to.
\\n
TRIUMPH-1 used a different, hybrid estimand for the knee pain and sleep apnoea outcomes, and also reported ITT results alongside it.
\\n
Company press releases often quote a third approach, an efficacy estimand, which estimates the result if everyone had stayed on the study drug. Those figures are typically larger.
\\n
\\n
When comparing headline numbers between sources, it matters which estimand each figure uses. The papers themselves are the reference for the primary analysis.
\\n
Regulatory status
\\n
Neither paper describes an approval, and the TRIUMPH-2 authors describe retatrutide as \"currently under investigation\". A query of FDA\\'s drug approvals data through openFDA on 22 October 2026 returned no match for retatrutide. That is a database check on one date, not a statement of FDA\\'s position. For how to weigh trial papers against press releases and regulator records, see our guide on how to read a research signal.
\\n
Research-grade retatrutide sold for laboratory use is not the investigational product used in these trials, and the trial results do not describe it.
\\n
Open research questions
\\n
\\n
How the results hold up in the remaining TRIUMPH trials, including those not yet published in full.
\\n
What longer follow-up shows about weight regain after the study drug stops.
\\n
How each of the three receptor actions contributes to the reported effects.
\\n
How the heart-rate increases reported in the phase 2 trial and the hypotension reported in TRIUMPH-2 develop over longer periods.
\\n
\\n
Limitations of the evidence
\\n
\\n
Both trials were funded by the developer, and several authors are company employees.
\\n
The results come from two trials in selected adult populations with defined entry criteria, and may not apply to other groups.
\\n
The subgroup outcomes in TRIUMPH-1 rest on smaller numbers (574 and 243 participants).
\\n
This summary is based on the published abstracts and registry records. Instead, the WADA Prohibited List captures experimental and research-grade peptides through two overlapping categories that apply both in competition and out of competition:
\\n
\\n
Category S0 (Non-Approved Substances). This catch-all category covers any pharmacological substance that is not addressed by another section of the List and has no current approval by any governmental regulatory health authority for human therapeutic use. That includes compounds in preclinical or clinical development, discontinued drug candidates and designer or research-only peptides. Starting with the 2022 Prohibited List, WADA added BPC-157 by name as an explicit example under Category S0 so that athletes and support personnel could find it in a text search of the document.
\\n
Category S2 (Peptide Hormones, Growth Factors, Related Substances, and Mimetics). Section S2.2 covers growth hormone releasing peptides (GHRPs), growth hormone secretagogues and releasing factors such as CJC-1295, ipamorelin, hexarelin, tabimorelin and tesamorelin. Section S2.3 covers growth factors and growth factor modulators affecting muscle, tendon or ligament protein synthesis and vascularisation, and explicitly names thymosin-beta4 and its derivatives, giving TB-500 as an example.
\\n
\\n
Because both S0 and S2 are \"at all times\" categories, anti-doping laboratories screen for these peptides in out-of-competition urine and blood samples as well as race-day or event-day samples.
\\n
How anti-doping laboratories detect BPC-157 and TB-500
\\n
Short linear peptides are cleaved rapidly by peptidases in blood and tissues, which means that the intact parent peptide may appear in urine only briefly after administration in animal or in-vitro models. Analytical chemists working in anti-doping control therefore map stable breakdown fragments (metabolites) and screen for both the intact chain and its diagnostic fragments.
\\n
\\n
\\n
\\n
Compound
\\n
Sequence / structure
\\n
Key analytical papers
\\n
Matrices studied
\\n
\\n
\\n
\\n
\\n
BPC-157
\\n
15-residue linear peptide (GEPPPGKPADDAGLV)
\\n
He et al., 2022, PubMed; awan / stable-isotope study, 2023, PubMed
\\n
Rat and dog plasma/urine; human liver microsomes and S9 fractions
\\n
\\n
\\n
TB-500 (thymosin beta-4 fragment)
\\n
N-acetylated heptapeptide Ac-LKKTETQ (or 43-residue parent)
\\n
Quantification and metabolite study, 2024, PubMed; Equine population study, 2025, PubMed
\\n
In-vitro incubations, rat biological matrices, equine plasma/urine
\\n
\\n
\\n
Multi-peptide panels
\\n
Peptidic and non-peptidic doping agents
\\n
Harmonized blood workflow, 2026, PubMed
\\n
Dried blood spots (DBS) and liquid blood
\\n
\\n
\\n
\\n
BPC-157 metabolism and mass spectrometry
\\n
In a 2022 preclinical pharmacokinetic study in rats and dogs (He et al., 2022, PubMed), researchers reported that intact BPC-157 had a short elimination half-life and was degraded into shorter peptide fragments and constituent amino acids. Building on that challenge, a 2023 study in Molecules (PubMed) used stable-isotope labelling combined with ultra-high-performance liquid chromatography and high-resolution mass spectrometry (UHPLC-HRMS) to characterise the in-vitro metabolic profile of BPC-157 for doping control, identifying characteristic N- and C-terminal truncation products that extend the detection window.
\\n
TB-500 and thymosin beta-4 fragments
\\n
The name TB-500 is used in research catalogues both for the full 43-residue protein thymosin beta-4 and for its synthetic N-acetylated active-site heptapeptide fragment, Ac-Leu-Lys-Lys-Thr-Glu-Thr-Gln (Ac-LKKTETQ). A 2024 analytical paper in the Journal of Chromatography B (PubMed) validated a UHPLC-Q-Exactive Orbitrap tandem mass spectrometry method to quantify TB-500 alongside its cleavage metabolites in in-vitro assays and in rats. In 2025, a study in Drug Testing and Analysis (PubMed) reported a population baseline and detection strategy for thymosin beta-4 in equine doping control.
\\n
More recently, a July 2026 paper in The Analyst (PubMed) described a rapid, harmonized extraction and mass-spectrometry workflow that screens both peptidic and non-peptidic doping agents in dried blood spots and liquid blood matrices.
\\n
How sport listings differ from medicine regulation
\\n
A listing on the WADA Prohibited List is a sport-governance rule, not a drug-approval record or a clinical trial finding:
\\n
\\n
WADA\\'s criteria. Under the World Anti-Doping Code, a substance is considered for the Prohibited List if it meets at least two of three criteria: potential to enhance sport performance, actual or potential health risk to the athlete, or violation of the spirit of sport. For Category S0, the absence of regulatory approval for human therapeutic use is sufficient to place a pharmacologically active substance off-limits in tested sport.
\\n
FDA\\'s records. In the United States, FDA evaluates bulk drug substances nominated for pharmacy compounding under section 503A. As covered in our report on the July 2026 FDA compounding committee, both BPC-157 and TB-500 were placed on the 23 July 2026 advisory committee agenda, and both appear on FDA\\'s page of bulk drug substances that may present significant safety risks among withdrawn Category 2 nominations.
\\n
\\n
For how to distinguish sport rules, advisory committee agendas and peer-reviewed trials, see our guide on how to read a research or regulatory signal.
\\n
Limitations of the evidence
\\n
\\n
Analytical doping-control papers establish how laboratories detect a compound and its metabolites in biological samples; they do not evaluate clinical safety or efficacy in humans.
\\n
Metabolic profiles generated in human liver microsomes, rat plasma or equine samples may not match human in-vivo urinary excretion kinetics in every detail.
\\n
Catalogue products sold as TB-500 may contain either the 7-residue fragment Ac-LKKTETQ or the 43-residue thymosin beta-4 sequence, so analytical studies specify the exact molecular mass and sequence tested.
\\n
\\n
Key terms
\\n
\\n
Category S0. The section of the WADA Prohibited List covering pharmacologically active substances with no current governmental regulatory approval for human therapeutic use.
\\n
Category S2. The section covering peptide hormones, growth factors, related substances and mimetics.
\\n
UHPLC-HRMS. Ultra-high-performance liquid chromatography coupled to high-resolution mass spectrometry, used to separate and identify peptides and their breakdown fragments by exact mass-to-charge ratio.
\\n
Dried blood spot (DBS). A sampling method in which small drops of blood are dried on filter paper for transport and mass-spectrometric analysis.
\\n
\\n
Where researchers source it
\\n
Purita is a store that lists BPC-157 for laboratory research use only. (1977) reported its sequence in PNAS, describing a heat-stable, highly acidic polypeptide from the thymus extract known as thymosin fraction 5. The authors linked it to the regulation and differentiation of thymus-dependent lymphocytes (T cells).
\\n
Later laboratory work, summarised in a historical overview by Garaci (2007), reported effects on MHC class I and Toll-like receptor expression and on cytokine production. Those findings framed thymosin alpha-1 as an immune-regulating peptide and led to trials in sepsis, where immune suppression after the initial inflammatory phase is a recognised research question.
\\n
The earlier signal: ETASS (2013)
\\n
The first sizeable randomised trial in sepsis was ETASS, published in Critical Care (Wu et al., 2013; NCT00711620). It randomised 361 adults with severe sepsis at six teaching hospitals in China between 2008 and 2010 to thymosin alpha-1 or a control group, alongside conventional care.
\\n
\\n
28-day all-cause mortality was 26.0% with thymosin alpha-1 and 35.0% in the control group.
\\n
The nonstratified analysis gave P=0.062 (log-rank P=0.049), and the relative risk of death was 0.74 (95% CI 0.54 to 1.02).
\\n
The thymosin group showed a larger rise in monocyte HLA-DR, a marker of immune function used in sepsis research.
\\n
\\n
The main comparison sat at the edge of conventional statistical significance, and the authors\\' conclusion was cautious, saying thymosin alpha-1 \"may be effective\" in a targeted population. TESTS, from an overlapping group of investigators, was the larger placebo-controlled test of that idea.
\\n
The TESTS trial in detail
\\n
TESTS randomised adults with sepsis to thymosin alpha-1 or a matching placebo for up to seven days, on top of standard care. The trial was funded in part by SciClone Pharmaceuticals, and some authors reported grants or consultancy fees from the company. The BMJ published a correction to the paper in May 2025.
\\n
How TESTS changes the pooled evidence
\\n
A meta-analysis published in September 2025 (Gu et al., Frontiers in Cellular and Infection Microbiology) pooled 11 randomised trials. Across all trials, the odds of death were lower with thymosin alpha-1 (odds ratio 0.73). When the analysis was limited to higher-quality trials (OR 0.82, P=0.09) or multicentre trials (OR 0.86, P=0.20), the difference was no longer statistically significant. A trial sequential analysis judged that the total sample was still too small to reach a firm conclusion.
\\n
This pattern is common in research: smaller and single-centre trials tend to show larger effects, and the estimate shrinks as larger, blinded trials are added. TESTS is now the largest placebo-controlled trial in this setting, and it carries much of the weight in any pooled estimate.
\\n
Regulatory status
\\n
\\n
United States. Thymosin alpha-1 is not an FDA-approved drug. FDA\\'s page on bulk drug substances that may present significant safety risks lists \"Thymosin-alpha 1 (Ta1)\". FDA states that compounded drugs containing it may pose a significant risk of immunogenicity for certain routes of administration and may have complexities with peptide-related impurities and characterisation of the active ingredient, and that the safety information is inadequate.
\\n
Sport. Peptides without approval by any governmental health authority fall under Category S0 of the WADA Prohibited List; see our report on the WADA Prohibited List and research peptides for how that category works.
\\n
\\n
For how to weigh a trial result against a regulatory record, see how to read a research or regulatory signal.
\\n
Limitations of the evidence
\\n
\\n
TESTS and ETASS were both conducted in China, so the results may not carry over to other health systems or sepsis populations.
\\n
Subgroup findings for age and diabetes come from a trial with a neutral primary outcome and need confirmation in a dedicated trial.
\\n
ETASS was much smaller than TESTS, and its primary result sat at the edge of statistical significance.
\\n
The meta-analysis combined trials with different designs, sizes and risk of bias.
\\n
The trial evidence concerns the pharmaceutical peptide in hospital research settings. It says nothing about research-grade material sold for laboratory use.
\\n
\\n
Key terms
\\n
\\n
Sepsis. Life-threatening organ dysfunction caused by a dysregulated host response to infection.
\\n
Modified ITT (mITT). An analysis of randomised participants that excludes a small, prespecified set, such as those who never received the study drug.
\\n
Hazard ratio. The ratio of event rates over time between two groups; 1.0 means no difference.
\\n
Monocyte HLA-DR. A cell-surface marker measured in sepsis research as an indicator of immune suppression.
\\n
Trial sequential analysis. A method that checks whether a meta-analysis has enough participants to support a firm conclusion.
\\n
\\n
Where researchers source it
\\n
Purita is a store that lists thymosin alpha-1 for laboratory research use only. (2025) reported a 68-week, double-blind, placebo-controlled trial (NCT05567796) in adults with a body-mass index of at least 30, or at least 27 with at least one weight-related condition, and without diabetes. A total of 3,417 participants were randomly assigned to CagriSema, placebo, cagrilintide alone or semaglutide alone.
\\n
\\n
Primary result: mean change in body weight from baseline to week 68 was −20.4% with CagriSema and −3.0% with placebo, an estimated difference of −17.3 percentage points (95% CI −18.1 to −16.6)
\\n
Adverse events: gastrointestinal adverse events were reported in 79.6% of the CagriSema group and 39.9% of the placebo group; most were described as transient and mild to moderate
\\n
Funding: the trial was funded by Novo Nordisk
\\n
\\n
A post hoc analysis of the same trial, published in Hypertension (Verma et al., 2026), reported a mean change in systolic blood pressure of −10.9 mmHg with CagriSema and −2.8 mmHg with placebo. A further post hoc analysis in Diabetes, Obesity and Metabolism (Busetto et al., 2026) looked at waist and other anthropometric targets. Post hoc analyses were not the trial\\'s primary question and are best read as exploratory.
\\n
REDEFINE 2: adults with obesity and type 2 diabetes
\\n
Davies et al. (2025), published alongside REDEFINE 1, reported a 68-week trial (NCT05394519) in 1,206 adults in 12 countries with type 2 diabetes and a body-mass index of at least 27, randomly assigned in a 3:1 ratio to CagriSema or placebo.
\\n
\\n
Primary result: mean change in body weight was −13.7% with CagriSema and −3.4% with placebo, an estimated difference of −10.4 percentage points (95% CI −11.2 to −9.5)
\\n
Glycaemic measure: 73.5% of the CagriSema group and 15.9% of the placebo group reached a glycated haemoglobin of 6.5% or less
\\n
Adverse events: gastrointestinal adverse events were reported in 72.5% and 34.4% of the two groups
\\n
\\n
As in other trials of this drug class, the weight reduction in participants with type 2 diabetes was smaller than in REDEFINE 1.
\\n
REDEFINE 5: a head-to-head trial in East Asia
\\n
Yamauchi et al. (2026) reported REDEFINE 5 (NCT05813925), a double-blind trial at 21 sites in Japan and one in Taiwan that compared CagriSema directly with semaglutide alone in 331 adults with overweight or obesity.
\\n
\\n
Primary result: mean change in body weight was −18.4% with CagriSema and −11.9% with semaglutide, an estimated difference of −6.5 percentage points (95% CI −8.4 to −4.6)
\\n
Why it matters for researchers: it is the first published REDEFINE trial with an active comparator as the main contrast, which isolates the added effect of cagrilintide in this population
\\n
\\n
Unlike REDEFINE 1 and 2, the REDEFINE 5 paper uses the trial product estimand as its primary analysis, which assumes the product was taken as intended. That figure is not directly comparable with the primary figures from the two NEJM trials.
\\n
REDEFINE 3: the cardiovascular outcomes trial
\\n
The newest entry in the record is a design paper rather than a result. Cavender et al. (2026), in the American Heart Journal, describe REDEFINE 3 (NCT05669755), which randomised 7,076 adults with established cardiovascular disease and a body-mass index of 25 or more, with or without type 2 diabetes, at 575 sites in 24 countries between January 2023 and December 2024. Mean age at baseline was 66 years, and 62% had a prior heart attack.
\\n
\\n
Primary question: whether CagriSema is non-inferior to placebo for three-point major adverse cardiovascular events (cardiovascular death, non-fatal heart attack or non-fatal stroke)
\\n
Secondary question: whether it is superior to placebo on a four-point composite that adds coronary revascularisation
\\n
Status: event-driven; no outcome results had been published at the time of writing
\\n
\\n
How this fits the wider pipeline
\\n
KlowSignal has covered retatrutide\\'s TRIUMPH phase 3 papers, which tested a single molecule acting on three receptors. CagriSema takes a different route, pairing two separate peptides, an amylin analogue and a GLP-1 receptor agonist, in one product. Direct comparisons between the two programmes are not possible from the published record: the trials differ in duration, populations, comparators and analysis methods.
\\n
Limitations of the evidence
\\n
\\n
Single sponsor: all four papers come from trials funded by the developer, with company employees among the authors
\\n
Estimands: REDEFINE 1 and 2 report their primary figures under an estimand that keeps every randomised participant in the analysis whether or not they stopped the product, while REDEFINE 5 uses the trial product estimand, so the headline figure depends on how discontinuation is handled; compare like with like
\\n
Duration: 68 weeks is long for a weight trial but short for questions about durability, and REDEFINE 3 has not reported
\\n
Generalisability: REDEFINE 5 enrolled only participants in Japan and Taiwan, so its results may not transfer directly to other populations
\\n
Not a research reagent: the trial product is a pharmaceutical formulation; findings say nothing about the identity, purity or behaviour of research-grade cagrilintide or semaglutide
\\n
\\n
Key terms
\\n
\\n
Amylin analogue: a peptide designed to mimic amylin, a hormone released with insulin by the pancreas
\\n
GLP-1 receptor agonist: a compound that activates the glucagon-like peptide-1 receptor
\\n
Non-inferiority: a trial design that tests whether a new product is not worse than a comparator by more than a pre-set margin
\\n
MACE: major adverse cardiovascular events, a standard composite outcome in cardiovascular trials
\\n
\\n
Record dates: 22 June 2025 (REDEFINE 1 and 2 online), 17 April 2026 (REDEFINE 5 online), 6 October 2026 (REDEFINE 3 design).
