FICTIONAL WORKED EXAMPLE
1. Need: Some patients leave a medical appointment unsure how to follow the
agreed care plan between visits.
2. Improvement: A planned opportunity to explain what is unclear and identify
the next conversation they need.
3. Team: A medical clinician and mental health professional explore follow-up,
with a named person to receive questions and clarify responsibilities.
4. Investigate: Define activities first; compare relevant service arrangements
and the practice's actual payer products. No arrangement is selected.
5. Missing facts: The clinical lead clarifies activities; the practice manager
checks available staff time; the billing lead locates applicable payer rules.
6. Conversation: Bring this sketch to the team to decide whether to develop a
fuller proposal and which questions to resolve first.
